Please call us or to schedule a new patient visit or email us. If you email us your name, insurance information, and phone number and we will call you to schedule an appointment. Please note that this request is only for new patient registration and not for patient care. Therefore, please do not send any documents or medical records at this time. If you prefer, you can also call us and we will do our best to assist you in scheduling a visit.
We understand the importance of providing high-quality care and consultation to our patients. Our goal is to make you feel as comfortable and welcome as possible, whether you are seeking treatment for an illness, injury, or simply maintaining your health. We believe in providing respect for your time, schedule, and vision for your own health.
Our approach to care is to not only address your current concerns but also to understand any potential future health issues that may be connected. This may require taking more time to understand the larger picture of your health and to create a comprehensive plan for your future.
We are pleased that you have chosen us as your primary care doctor and suggest scheduling an annual physical examination to establish a complete database of your health. We recommend that you bring a copy of your last labs and any pertinent records, such as recent MRI's, CT's, x-rays, colonoscopies, etc. Additionally, we ask that you bring all of your medicine bottles or a photo of each bottle, any home blood pressure readings, and a copy of your immunization records.
We are able to arrange any additional blood or urine tests and schedule X-rays and ultrasound studies at your convenience. Please note that some screening studies may not be covered by your insurance and we will inform you of which studies are likely not to be covered. Medical examinations are not foolproof and cannot guarantee protection from all illnesses, but investing in your health today with this type of evaluation and continuity of care is likely to improve your quality of life.
Please bring a copy of your insurance card and be prepared to pay for any co-pay or deductibles that your insurance requires. We ask that you arrive at least 15 minutes prior to your appointment.
We use a patient portal for our patients to update demographics, pay bills, view lab results, request refills, and send messages to their provider. We will set you up on the portal during your first visit. If for some reason your doctor is unavailable, we have mid-level providers and a Plano office that can also assist with your needs.
